Instructions for using the service

Entering incorrect data will result in the cancellation of your request to open or renew the medical file - make sure that the entered data is correct and complete.
In the event of renewal, the eligibility for treatment must have already expired or be less than a month away from expiring.
The images of the uploaded documents must be clear so that the request is not rejected.
The uploaded documents must be identical to the original and the owner of the medical file bears full responsibility for uploading any forged or non-identical documents.
The medical file opening form / medical file renewal form for military personnel on duty must be dated recently.
A request to open / renew each medical file must be submitted as an independent request.
