Obstetrics & Gynecology Department

Introduction

  • Armed forces Hospitals program, Southern region is serving a big sector of military population in the kingdom. It is a referral center to all other military hospitals in the southern region. King Faisal military hospital is a part of armed forces hospitals program. It includes obstetrics & gynecology department, neonatal intensive care unit, family medicine and dental clinics. Obstetrics and gynecology is the biggest department in the king Faisal hospital. It includes (140) beds distributed over six wards.

Emergency Room(ER):

  • Four consultation rooms are available, supported by ultrasound machines.
  • Two Delivery beds supported by all delivery instruments, CTG and neonatal resuscitation unit . A midwife is available 24 hours in ER to help any woman coming in late labor to ER.
  • Two observation rooms with 8 beds.
  • Trauma area includes 4 beds, supported by ECG machine and first aid equipments.
  • Since 2014, a Sonographer is available in ER over 24 hours to help in emergency ultrasound scanning.

Labor ward (include four areas):

  • Early labor ward (6 rooms), each room supported by CTG machine.
  • Active labor ward, (7 rooms), each room supported by CTG machine, a midwife for each room 24 hours running for vaginal delivery.
  • Fourth stage ward, (6 rooms), for close observation of post vaginal delivery cases. Each room for all previous wards is supported by a separate bathroom.
  • The ward for induction of labor, (7 Beds) each room supported by CTG machine.

Postpartum ward:

  • It includes 22 beds in addition to fully equipped isolation room infectious cases. Women who delivered vaginally or by caesarian section are admitted to this ward for care. The ward is supported by nursery to manage stable neonates, keeping them beside their mothers to help breast feeding.

Antenatal ward:

  • This ward is newly opened this year, 2015. It includes 36 beds, 24 of them are active now & it is planned to work by full capacity within the current year. It give care for cases of high risk pregnancy. It is supported by electronic machines to monitor fetal and maternal health.

Gynecology ward:

  • This ward includes 27 beds, it helps for preoperative preparation and post operative care for gynecologic cases in addition to patients who needs for admission for early pregnancy problems as, missed abortions, ectopic pregnancy, threatened abortions, etc…. An isolation room has been established in the Gynecology ward.

High Dependency Unit(HDU):

  • The Obstetrics and Gynecology department is currently dealing with very high risk patients who need close monitoring. High Dependency Unit consists of one intensive care bed with 10 high dependency beds fully staffed from nursing side. Clear guidelines and management protocols are followed for this unit.

Operation Room(OR):

  • This part of the hospital includes 3 operations rooms and recovery area. Both emergency and elective , major and minor cases are managed in this OR. It is well equipped with endoscopy instruments, facilitating both hysteroscopic and laparoscopic surgery. The hospital is processing to have a new theatre for emergency cases connected to labor ward and renovation of the current theaters to be supported with electronic devices (OR-1) to facilitate more advanced surgery, specially endoscopic. Work started already and planned to be finished by the of the current year, 2015.

Outpatient department (OPD):

  • Around 400 women are daily reviewed in Obstetrics & Gynecology clinics (105,124 per year): according to last statistics. This huge number is now served by two different medical teams (one for Gynecology and one for Obstetrics). Eleven rooms are running daily and every room works in two sessions (morning and afternoon). For the last 3 years, infertility clinic is now held on daily basis and under supervision of the consultant.

Ultrasonography and CTG services in the clinic:

  • CTG service in clinic is now served with 4 beds equipped with 4 CTG machines. The Ultrasonography service became available 24 hours every day. The unit conducted around 5000-6000 scans monthly for the out-patients and in-patients. ER, OPD, OR, Antenatal, Labor & Delivery, and Gynecology wards are also provided with separate ultrasound machines to cover the emergency needs for performing bedside ultrasound scans. New 3D/4D ultrasound machines were installed in the unit and used as tertiary scan for high risk patients and those with fetal structural anomalies. Daily fetal anomaly scan clinic is carried out by ultrasound unit supervisor. These cases are revised by consultant radiologist to confirm the diagnosis and thus achieving a multi-disciplinary approach. Since then, the number of cases with undiagnosed fetal anomaly was reduced and patient's satisfaction improved.

Conferences and workshops:

  • Since 2010, the Obstetric and gynecology department is regularly organizing conferences, and workshops.
  • 2010: Reproductive health: Lessons from the past and hopes for the future (international conference). Advances in the management of obstetric hemorrhage.
  • 2011: Urogynecology updates (conference)
  • 2012: Basic gynecologic endoscopy workshop. Gynecologic endoscopy updates (conference) Basics of obstetrics and gynecology ultrasound workshop. International infertility conference; New horizon in management of infertility.
  • 2013: The Impact of Genital Infection on women health (conference). Updates on contraception (conference).
  • 2014: Advances In Gynecologic Laparoscopy Workshop. Updates In Hysteroscopy Workshop.

Saudi Board training program:

  • This is the sixth year after starting the program in the Obstetrics & Gynecology department; our database now contains 19 trainees from Saudi board training program; 4 trainees in R1, 4 trainees in R2, 5 trainees in R3, 4 trainees in R4 and 2 trainees in R5. In 2014, 4 trainees (among 5) have been certified by Arab board and one trainee has been certified by Saudi board. They are receiving weekly teaching session for 4 hours in addition to clinic and operative training.

Future plan of obstetrics & gynecology department

  • Implementing 2 operative wards, one for emergency & the other for elective cases, fully equipped by electronic devices and advanced endoscopic surgery. (in process)
  • Renovation of the outpatient department (started)
  • Start outpatient endoscopy unit including colposcopy and hysteroscopy. This will help early accurate diagnosis of lower genital tract malignancy and hence, treatment.
  • Outpatient hysteroscopy will minimize admissions of the indicated cases that will be cost effective for the hospital and satisfactory for the patients.
  • Urogynecology clinic, equipped by urodynamic unit.
  • Unit for assisted reproductive technology (ART).
  • Start full capacity of the antenatal ward.
  • To run epidural analgesia/ anesthesia in labor ward to reach the highest level of patient satisfaction and minimize the rate of caesarian section on maternal request.
